Abstract
A 30-year-old young male presented with alleged history of firecracker injury 5 months ago and was diagnosed with right eye traumatic cataract with fibrosed anterior capsule and complete posterior synechiae with BCVA of counting fingers at 50cms. He was taken up for right eye phacoemulsification. Synechiae hampered in pupillary dilation and were lysed using vannas scissors and iris repositor to enhance visualization of the lens. Continuous curvilinear capsulorhexis of around 4 mm was attempted using 26 G needle cystitome and extended using utrata forceps. Some of fibrosed capsule was cut using vannas scissors. Phacoemulsification was performed using continuous irrigation and low aspiration rate(20 cc/min) and low vacuum(200 mm Hg). Irrigation and aspiration was done using vacuum of 450 mm Hg. PCIOL was implanted and dialed. Patient was followed up postoperatively and his BCVA improved to 6/18 after 1 week. This is how a challenging traumatic cataract was dealt with.
