Abstract

Methods: A 34 year old male patient was diagnosed with nuclear cataracts in both eyes; right eye was operated previously three years back, now posted for left eye temporal SICS with PCIOL. Biometric measurements included Kv 46.00 and Kh 47.50 with axial length of 22.32 mm and IOL power of 23.5 D (A constant-118.8). Under peribulbar anaesthesia a sclerocorneal tunnel constructed and phacodonesis observed during capsulorrhexis, hence can opener capsulotomy performed. Nucleus delivered by viscoexpression after hydrodissection and hydrodelineation. Inherent Capsular dehiscence noted at the lens coloboma site. Cortical clean-up done and PC rigid IOL implanted.

Results: Patient gained CF 3 meters on first post-operative day

Conclusion: We experienced intraoperative hypotony, phacodonesis, and difficulty in performing capsulorrhexis especially at the lens coloboma location and incomplete hydrodissection and hydrodelineation due to lens notching.
