Abstract

Third nerve palsy are challenging to treat. The only functioning muscles in a 3rd nerve palsy is Lateral Rectus and Superior Oblique. This leads to large exotropia and hypotropia with restriction of movements in all the gazes except abduction.

Various surgical options like periosteal fixation of lateral rectus, globe fixation of medial rectus have varying success. One of the innovative surgery is to transpose the lateral rectus to medial rectus. Now the abductor becomes an adductor and not only brings the eye in midline but makes sure that the long term results are also predictable and there is no exo drift.
