Abstract
A 34 year old male with Pellucid Marginal Corneal Degeneration (PMCD) visited our hospital with complains of sudden whiteness, pain, reduced vision in the left eye since 10 days. Visual acuity of the patient was CF at 1 metre in left eye. Left eye shows corneal edema due to acute hydrops localized to inferior three-fourth of the cornea and beginning of inferior vascularization. Anterior segment OCT showed increased corneal thickness due to stromal edema and a large descemets detachment. The placement of compression sutures was based on pre-op anterior segment OCT, an endoilluminator was used to help demarcate the descemets membrane. A triangular descemets tear was noted at 6 O’Clock position above area of thinning and 3 full thickness compression sutures were placed with 10-0 prolene suture across the tear including about 1mm from the edges of the tear. This was the first case in literature of hydrops in PMCD managed managed successfully with endoilluminator assisted corneal suturing
