Abstract
A 65 year old female patient came with complaint of sudden loss of vision in left eye for 4 days with no significant history. There was no history of systemic risk factors. On examination, fundus looked normal. Color vision was normal and central fields were not reliable due to low vision. Patient was taken for SD-OCT retina, which showed thickening of inner layers of retina with foveal involvement. Two diagnoses were considered- PAMM(Paracentral acute middle maculopathy) and CRAO(central retinal artery occlusion). Points favouring PAMM were only inner layers of retina were involved, with sparing of outer retinal layers. But as fovea is involved, this opposed the OCT criteria for PAMM. Points favouring CRAO were thickening of retinal layers and foveal involvement. But as outer retinal retinal layers have been spared, this point goes against case being labelled as CRAO. Patient was treated as CRAO and was given T.Acetazolamide and goniomassage.Patient was asked for follow up of 1 week.
