Abstract
The calculation of IOL power in post refractive surgery eyes is difficult. And the IOL power calculation
in eyes undergoing Radial keratotomy(RK) and Laser assisted in situ keratomileusis(LASIK)
simultaneously is even more tricky. A 43 years male, underwent RK in both eyes, followed by LASIK
for the residual power, was diagnosed with cataract in both eyes. The keratomteric(K) readings
unrecordable on optical biometry, showed wide variation on pentacam due to flat K. The IOL power
varied from +17 D to +22 D on the ASCRS calculator. The IOL power was calculated using the average
central corneal power, and adjusting the axial length by the Wang Koch method, and using it in the
Double K method. Post operatively, BCVA improved in right eye from 0.6 logmar to 0.5 logmar and
left eye from 0.5 logmar to 0.3 logmar .The assessment of new keratometric readings, optimisation
of constants and adjusted axial length measurements are the key to accurate IOL power calculations.
