Abstract

Aim: to present successfully managed cases of refractory ocular hypotony with an encircling anterior 240 band

Materials & methods:6 patients 5 male & 1 female with post blunt trauma unresponsive hypotony were thoroughly evaluated & no definitive cause was found in any of them. After 6 weeks of topical cycloplegia + steroids in all & anterior subtenons tricort in 2 patients proved ineffective, it was finally decided to try out an anteriorly placed 240 silicone band as a last throw of the dice in all patients after clearly explaining the likely outcomes

Results successful relief of hypotony was achieved in all patients with IOP improving to 16 mm Hg by the 4th postop day in all patients. 2 patients developed secondary glaucoma by the 6th day that was controlled with medications. No long term sequelae was noted after followup of an average period of 6,3 years

Conclusions: the above procedure constitutes an elegant & safe method of reversing recalcitrant hypotony being extraocular
