Abstract

Reconstruction of full-thickness upper eyelid defects for optimal cosmetic and functional outcome is challenging. This video shows important steps in the surgical management of large upper eyelid defects, following excision of large basal cell carcinoma involving the whole of the upper eyelid, with a modified Cutler-Beard technique. Cutler-Beard technique is a classic two-staged procedure for upper eyelid defect involving more than 2/3 of lid, utilizing full-thickness vertical advancement bridge flap from the lower eyelid.

Modifications in the technique that can give excellent results include: Excision of Burow’s triangle at both the ends of advancing flap. It helps in removal of tension in the advancing flap. This modification of Cutler -Beard technique is helpful in repair of large eyelid defect specially in younger patient where laxity of skin is less.
