Vitreoschisis is lamellar separation of the posterior vitreous cortex. This is due to anomalous Posterior vitreous detachment and is implicated in various vitreo-retinal interface disorders. This invisible danger is the frequent cause of adverse outcomes in surgery for high myopes and in patients undergoing surgery for vitreous hemorrhage or tractional detachment in diabetics. Another situation where it is of importance is during vitrectomy for retinal detachment. Frequently some vitreous remnants are seen and occasionally the vitreous cortex. Detecting this will prevent recurrent detachment.
This video demonstrates the challenge in detecting vitreoschisis during surgery and elegant methods in detection and removal of the vitreous cortex.
