Most often vitrectomy is unimanual the other hand providing illumination. The chandelier illumination provides an opportunity to go bimanual which is indispensable in separating adherent membranes from the underlying retina in challenging situations like Diabetic tractional retinal detachment, Tractional detachment in FEVR, Densely adherent membranes in retinal detachment with PVR and re-surgeries in diabetes with residual tractional membranes. Bimanual surgery is also useful in controlling bleeding during diabetic vitrectomy and removing subretinal bands.
