Abstract
Bimanual 25 gauge vitrectomy was performed in cases of morbid PDR with tractional retinal detachment , associated macular hole and combined detachments. Pre-operative swept source OCT along with intraoperative correlation helped demonstrate the importance of understanding vitreoschsis. This was useful to achieve the correct plane of dissection in cases of severe PDR presenting in varied forms of fibrovascular proliferations and tractional detachments. Tips on bimanual dissection for atraumatic separation of membranes and precise coagulation of bleeders along with tricks to handle associated macular holes in these morbid cases are depicted in the video.Preoperative OCT analysis for detecting vitreoschisis is helpful to achieve the correct plane of dissection. Bimanual MIVS aided in the atraumatic dissection. These tricks helped to ensure good anatomical and visual outcomes in cases of severe PDR
