Abstract
Once the tear has passed into the zone of the zonular fiber attachment near to the equator, the radial force that is applied to “bring back the rrhexis” may no longer work well to bring it back in; the force now also gets transmitted through the zonular fibers towards the ciliary processes and can lead to “disinsertions” over there with “zonular Dialysis”, with inevitability of vitreous phase disruption.
We present an innovative intervention, of cutting off the zonules (by a needle) to free them from connections to anterior capsule centrally; the transmission of force towards ciliary processes to initiate “zonular Dialysis” is minimized and now possible to bring back in, the path of the rrhexis.
