Abstract
Patient presented with history of cow horn injury in LE. LE revealed PL +Ve and PR accurate. Ant seg examination revealed subluxated lens & fundus could not be visualised. USG B scan was in conclusive. Subluxated lens was removed through a sclero-corneal tunnel with help of an AC maintainer. 3,25G ports made 3mm from limbus. Inside Vitreous cavity nothing could be visualised except clotted blood & everything is bare choroid. A small pyknotic mass of retinal tissue mixed with blood could be visualised. Vitrectomy done to remove blood & separate retina. Endodiathermy done around the edges of retinal break. After lot of dissection posterior pole could be visualised. PFCL injected to stabilise the posterior pole. Subretinal fibrovascular bands are removed. BBG injected under PFCL & ILM peeled off. Retina settled. EL done. PFCL air exchange done. Silicone Oil injected. Vitrectomy, with PFCL, membrane peeling, EL & silicone oil tamponade can bring fruitful results in complicated cases.
