Abstract
Macular holes larger than 2500 micron base diameter seldom tend to close, even with surgery. Various methods have been described in the literature for the same. We present here a case of a 55 year old housewife who came to us with complaints of decreased vision in right eye for 1 year. On examination, a full thickness macular hole was noted with a base diameter of more than 3000 microns. We planned a procedure wherein after vitrectomy, a large graft of ILM was taken from the macular area surrounding the hole and was gently tucked inside the hole, making sure its base or edges are left undamaged. Post op 3 months, not only did the hole close well anatomically but there was a functional improvement in the visual acuity to 6/36. This surgery demonstrates the novel method of taking grafts of ILM over the macula with the help of finess loop and technique of how to tuck the graft inside the hole like an origami using ILM peeling forceps.
