Abstract

We report a case of 13 years old boy who presented with sudden painless blurring of vision in right eye for 3 days. On examination right eye was perception of light (PL) negative , positive relative afferent pupillary defect , gross optic disc swelling and left eye was 6/6 N6 with normal fundus. Neuroimaging (MRI) Brain and orbit came to be normal. After Intravenous loading dose of solumedrol and oral steroids right eye vision improved to 6/6P ,but again deteriorated to counting finger close to face after stopping oral steroids. Further investigations were sent and hypothyroidism was diagnosed. Again high dose oral steroids were started along with drugs for hypothyroidism after physician’s opinion. Vision improved to 6/9 and maintained.

Our case report may suggest the possibility of sight threatening optic disc involvement with hypothyroidism in a child and shows us the importance of early diagnosis and intervention in a recurrent and even in a PL negative case.
