Abstract

Traumatic subdural hygroma (TSHy) is an accumulation of cerebrospinal fluid (CSF) in the subdural space after head injury. Subdural hygroma can be caused due to trauma or rarely can be secondary to rupture of asymptomatic arachnoid cyst secondary to trauma. It can act as space occupying intracranial lesion Initial hygromas elevate the intr-acranial pressure, which compresses the transverse sinus, further worsening the pressure.

A 26-year-old male came to the Ophthalmology OPD with complaints of diminution of vision in the both eyes and diplopia since 1 week which was sudden in onset, progressive in nature. He gave history of RTA 2 weeks ago. Visual acuity in the Right Eye was 6/12 and Left Eye was 6/9 and contrast sensitivity was reduced. Anterior segment was normal. Fundus examination showed papilledema. B scan showed increased optic nerve head diameter. Surgical management was done by doing guarded lumbar puncture to decrease ICP . Post procedure vision and diplopia improved.
