Abstract
56 year old male with no known co-morbidities presented with altered colour perception and reddish colouration of lateral visual field in right eye noted since past 5 days. No h/o trauma, flashes, pain or straining. Patient was evaluated in detail. General examination showed severe pallor, glossitis. Systemic examination was normal. Ophthalmic examination showed a vision of 6/36 not improving in right eye, 6/9 improving to 6/6 in left eye. Colour vision grossly defective bilaterally. Anterior segment examination normal, dilated fundus examination showed bilateral superficial and deep retinal hemorrhages mainly in the posterior pole, with a macular hemorrhage in the right eye. IOP was 9.5 and 12.5 mm Hg in right and left eye respectively. A differential diagnosis of blood disorders or systemic immunodeficiency disorder was made, patient was evaluated in detail and referred for medical consultation. Results confirmed acute myeloid leukemia and treatment was initiated.
