Abstract
Encephalocele is herniation ofbrain tissue through a defect in the skull base. Traumatic orbital encephaloceles are very rare(15 cases published).24 year old boy presented with complaints of “unable to see top part in right eye” .Visual acuity was 20/20 n6 Anterior segment was unremarkable.Both pupils equal &reactive. Applanation tonometry was 14 mm hg in both eyes.Fundus examination in right eye showed temporal pallor of optic disc.Left eye the fundus was normal.He had uneventful head injury from bike accident 3 months ago.. Humphrey field analyser (30-2) showed superior field defect ( VFI 56%) field right eye, left eye was normal. Cranial CT scan showed fracture of right frontal sinus with focal herniation of brain parenchymal tissue. Orbital coronal & axial scan corroborated the frontal encephalocele. Prompt neurosurgical repair prevented life threatening complications. Thus missed frontal encephalocele was diagnosed because of atypical presentation of field defect.
