Abstract
44 year old male presented to our outpatient department with complaints of painless progressive loss of vision in Right eye for 3 days. Best corrected visual acuity was 6/36 in right eye and 6/6 in Left eye. Intraocular pressure and anterior segment examination was within normal limits. Fundus examination revealed diffuse white centered hemorrhages (Roth spots) in both eyes with a small Pre-retinal hemorrhage over the macular area in right eye . Complete blood count was normal except Hemoglobin to be 5.9 gm/dl . Renal parameters (Urea and creatinine) were deranged. Peripheral smear revealed normocytic normochromic anemia. Serology was negative for HIV. The patient was referred to a cardiologist and nephrologist for further management.
Roth spots has a wider implication and can be a window to a serious systemic ailment which has to be diagnosed and addressed.
