Abstract
Purpose: This study describes the long-term outcomes of DSAEK in the management of CHED
Methods: 174 DSAEKs were performed on 174 eyes of 110 subjected to visually significant endothelial dysfunction due to CHED between 2008 and 2021. Results: Median age at DSAEK was11 years (IQR, 6.6 to 17.4 yrs.). At a median post-interventional follow-up of 2.7 years (IQR, 0.6- 4.8 years). 87.1% (149 of 171 eyes) maintained a clear graft. Median BCVA improved from 1.39 ± 0.05 preoperatively to 0.85 ± 0.05 at the last follow-up visit (p<0.0001). The cumulative overall long-term graft survival was 96.8%, 87.1%, and 76.7% at 1, 3, and 7 years respectively. Conclusions: In CHED, DSAEK is a safe, effective, and superior surgical alternative to full-thickness keratoplasty. This study’s outcomes suggest that DSAEK has a low postoperative complication rate and excellent long-term graft survival even after a decade.
