Abstract

A 55yr old male patient,20 days h/o sudden painfull loss of vision, redness and swelling in right eye. Alcoholic 25 yeras .Nil systemic.O/E RE-NPL, lid edema, conjunctival chemosis& SCH,cornea heazy,AC shallow,NVI,VH.IOP-36 mmhg. Fundus-VH.LE -pseudophakia, 6/36.AS-NORMAL,IOP16mmhg.Fundus examination shows laser spot. Based on history and examination possibly ocular unnoticed traumatic subconjunctival haemorrhage with vitreous haemorrhage suspected,used to work under influence of alcohol &might have missed trivial trauma and patient was evaluated.B scan-RD WITH VH.Rx-antiglaucoma medicatication,antibiotic e/d& IV antibiotic. Routine Ix-Hb 15.2gm/dl, TLC 8,800/cumm and platelet count was 8000/cumm.Other- normal.Severe thrombocytopenia cause of SCH with VH without history of trauma, no other bleeding manifestation.

Conclusion-chronic thrombocytopenia, retinal neovascularisation withVH & SCH causing loss of vision,importance of reviewing history if not matches with examination findings.
