Abstract
A 57-year-old male presented with complaints of pain and redness in right eye of 4 days duration. Past medical records showed thymectomy 3 years ago for thymoma, followed by 3 attacks of bronchopneumonia, recent being 1 week prior to ocular symptoms. On examination he had corneal edema with raised IOP. AC cells 3+ were noted with vitritis and retinal lesions suggestive of CMV-retinitis. His leucocyte count and gammaglobulin levels were reduced and serology negative for HIV, HbSAg and HCV. PCR confirmed the diagnosis, and started on oral Valgancyclovir 900mg BD for 1 week. At 1 week follow up retinal lesions remained same in size with granular appearance, and no improvement in visual acuity. Intravitreal Gancyclovir 2.5mg was given In addition with oral Valgancyclovir. At 1 month post injection retinitis was resolving and oral medications continued. A rare variant of thymoma causing immunosuppression in adults due to hypogammaglobulinemia known as ‘Good” syndrome has been described.
