Abstract
Purpose: A 40 year old male presents with unilateral loss of vision in left eye for 2 days.
He complain of sudden vision loss in left eye noticed after waking up in morning.He is a known case of hypertension for last 10 years and taking medications irregularly .On examination vision in right eye is 6/6 while in left eye finger count at 2 meter unaided which was not improved with refraction and his blood pressure was 190/110mmHg.Anterior segment examination was normal in both eyes and on fundus examination right was normal while in left eye tomato splash appearance with tortuous vessels of CRVO with reddish patch of macular hole was present which was later confirmed on OCT.In OCT Macula right eye was normal while in left eye macular hole was present without and edema.Patient was planned for surgery and he underwent macular hole surgery with gas tamponade.Unfortunately even after the operation he only gained 6/60 vision in his left eye in follow up and haemorrhages of CRVO was resolved.
