Abstract

Aim: We discuss a case of 54-year-old female with bilateral primary angle closure (PAC) and rheumatoid arthritis with scleral thinning. Methods: She had DOV BE since 4 months. She was K/C/O RA and on Tab. HCQ 200 mg HS since 15 years. She was on Dorzox-T and Pilocar EDS BE since 16 years from outside hospital. Her BCVA was 6/24p in RE and 6/12p in LE. IOP was 36 in RE and 32 mmHg in LE. SLE showed diffuse scleral thinning, hypochromic iris, patent Laser PI, immature cataract in BE. LT of 4.94 and 4.91 mm. Occludable angles in BE. Cup-disc ratio of 0.5 BE. She was diagnosed with BE PAC with IMC. 2 AGM was added in BE. She underwent GATT+ PHACO in BE 1 month apart RE f/by LE. Result: At the end of 2 months post op of BCVA was 6/6P in RE and 6/6 in LE. IOP was 15 in RE and 13 mmHg in LE. Conclusion: GATT + PHACO is a good alternative for patients who have contraindication for penetrating glaucoma surgeries.
