Abstract

An 82 year old female reported with severe pain and redness in the right eye since 3 days . The BCVA in the right eye was counting fingers close to face in the right eye and the IOP was 52 mm Hg. Slit lamp examination showed severe conjunctival and ciliary congestion, diffuse microcystic corneal edema, with a flat anterior chamber and occlusio pupillae in the right eye. Left eye was pseudophakic with BCVA of 6/6. Systemic and topical anti glaucoma medication along with intense topical steroids were administered. Nd YAG laser was used to perform a large iridotomy centrally to create a passage for fluid into the anterior chamber which would also serve as a pupil thereby improving vision. Immediately after the procedure falling back of the iris was seen. On follow up, the BCVA improved to 6/36, with IOP dropping to 28mmHg and the AC was deep, with relief of symptoms.
