Abstract
The most common manifestation of ocular tuberculosis is choroidal granuloma. These lesions respond well to conventional antitubercular treatment. But very rarely vascularized granuomas may need adjunctive therapy if resistant to conventional treatment.
We report a case of a 23 year old young female with best corrected visual acuity in right eye 6/6 and left eye 6/12 had a choroidal granuloma in her left eye very close to fovea with adjacent exudation of tuberculous etiology; not responding to conventional treatment with oral steroids and anti-tubercular treatment drugs and needed additional intravitreal injections of bevacizumab. After two doses of intravitreal injections the lesion responded well with dramatic decrease in its size along with complete resolution of surrounding exudation.
Intravitreal anti-VEGF injections should be considered as an adjunct in large vascularised granulomas if not responding to conventional anti tubercular treatment
