Abstract

Topiramate is an oral sulphamate medication commonly prescribed by neurologists for migraine, epilepsy and neuropathic pain. It occassionally precipitates secondary angle closure attacks, disturbance in the refractive error and intra-ocular pressure. We study a case series of 8 patients who presented in last 1 year with the complaints of acute diminuition of vision, complaints of seeing halos, raised intra-ocular pressure. On examination, they all had secondary angle closure attacks with a common history of taking topiramate. We also investigated them on AS OCT where we saw the angle reduced, decrease in AC depth and sudden appearance of refractive errors. The treatment was varied depending on the degree of IOP rise. Commonly, on cessation of Topiramate, all parameters returned to near normal values after 3 days. Topiramate, hence, must be judiciously prescribed and investigated if encountered.
