Abstract
Purpose: Severity of thyroid eye disease (TED) is decided as per EUGOGO guidelines which includes lid retraction, proptosis, diplopia & soft tissue signs. TED is shown to have retrobulbar hemodynamic changes, which may provide an accurate disease monitoring and help predict onset of dysthyroid optic neuropathy (DON).
Methods: 33 eyes of moderate-to-severe TED, 15 eyes of mild TED, and 18 eyes of healthy controls underwent colour doppler imaging of ophthalmic artery (OA) and central retinal artery (CRA). The peak systolic velocity (PSV), end diastolic velocity (EDV), an resistivity index (RI) were evaluated. Area under curve (AUC) analysis was done.
Results: AUC analysis showed maximum discrimination power of OA-EDV to discriminate healthy vs TED orbits. However, severity of disease was better predicted by OA-PSV. OA-RI and CRA-RI had little role in predicting severity of TED.
Conclusions: Arterial velocities alter with progression of TED. The resultant orbital ischemia can lead to DON.
