Abstract

A 25 years-old apparently healthy lady was transfused 4 units of platelets (outsourced) after she developed thrombocytopenia on day 2 of normal vaginal delivery at a tertiary care hospital. Antenatal period was uneventful.12 hours following transfusion she developed rapidly progressing redness, pain, DOV, proptosis OS. VA was PL with inaccurate PR due to orbital cellulitis with endophthalmitis. VA OD- 6/6. Cornea OS was hazy due to high IOP and ring stromal infiltrates. CECT confirmed clinical diagnosis & cavernous sinus thrombosis (CST) noted. Conjunctival discharge showed pus cells. Blood & urine cultures were sterile. Gynaecological assesement unremarkable. A possibility of endogenous bacterial dissemination from transfused blood products &/or intravenous catheter insertion site considered. Conservative management resulted in amelioration of proptosis, CST and clinical stabilisation. This unusual case highlights risks involved in transfusion of possibly contaminated blood products.
