Abstract

Secondary angle closure Glaucoma due to uveal effusion and forward rotation of ciliary body has been reported in numerous drug induced ,systemic and ocular conditions.

This is a rare case where a 19 year old female presented with, Bilateral ACG and ciliochoroidal effusion as an initial manifestation of SLE , associated with lupus related pleural effusion, ascitis and medical renal disease.

She presented with Acute onset of defective vision -OU,with a BCVA – 6/6 with myopic correction of -4D, IOP- OD-24 & OS-22 mm hg. Gonioscopy showed 360 deg appositional closure .She also had Angioedema and Skin Rash. Further- B scan ,UBM, FFA were done to confirm the diagnosis. Systemic investigations – blood titres of ANA,USG – abdomen , Chest CT confirmed the diagnosis of SLE according to ACR criteria.

She was treated with topical AGM ,systemic steroids and immunosupressants.

Hence such meticulous systemic evaluation helps in prompt referral for timely treatment of the underlying disease.
