Abstract

In this study, inferior rectus (IR) recession was done as first procedure in ten diagnosed cases of MED irrespective of forced duction test (FDT) finding. Out of the ten patients, six had positive FDT for IR. Mean preoperative vertical deviation was 23.8 prism diopter (PD)(20-50PD). All patients underwent IR recession 6-6.5mm with conjunctival recession. Seven patients were orthophoric. Three required secondary procedure: one was subjected to contralateral superior rectus resection and two underwent Knapp’s procedure. Final mean post operative vertical deviation was 1.8 PD (0 PD-5 PD). Thus, IR recession should be first procedure of choice in cases of MED, if deviation is less than 25PD. This surgery can also be considered as first procedure in cases with larger deviation(45-50PD) followed by Knapp’s procedure if required irrespective of FDT findings.
