Abstract

57/M presented with left eye swelling, redness, eye pain and pus draining from infra orbital region. H/o injury with wooden piece present, while cutting wood, which occurred one week back.

General examination normal. Left eye showed diffuse periorbital tense swelling of left eye with severe conjunctival congestion and chemosis, with pus draining from infraorbital region. There was a 6/18 vision. Extra ocular movements showed gross restriction and painful eye movements.

Wound exploration was done extensively and 3 wooden pieces were taken out, IV antibiotics given.

Retained wooden foreign body in orbital, periorbital region is very dangerous, as it is a good culture medium due to its organic nature, and can be missed even by a CT or MRI imaging. Even little retained particle can cause severe ocular/intracranial infection (bacterial / fungal), abscess and foreign body granuloma. So early and aggressive wound exploration, abscess drainage, imaging and iv antibiotics should to given.
