Abstract

A 16-year-old female presented with defective vision in the right eye for the past 2 days. Best corrected visual acuity was 6/18-right eye and 6/6-left eye. In the right eye, the anterior chamber had +1 cells. Clinically, evident serous retinal detachment involving the macula was visible on dilated fundus exam & documented with OCT in the right eye. The left eye was unremarkable except for occasional cells in Anterior chamber. Within 1 week , the left eye showed signs of involvement with BCVA of 6/9 with retinal findings similar to the right eye. Investigative panel were within normal limits except for IgM which was equivocal for toxoplasmosis. Based on this, a diagnosis of atypical ocular toxoplasmosis with serous retinal detachment was made. The patient was managed with oral Azithromycin and oral steroids. Two weeks after initiating treatment, the clinical presentation improved with complete resolution of subretinal fluid and restoration for vision to 6/6 in both eyes.
