Abstract

A 44-year-old male, not a known case of any medical illness presented with complaints of blurred vision and floaters in his right eye since 5 days. History of hard of hearing in the past 20 years following childhood trauma. His best-corrected visual acuity was 6/12 in right eye and 6/6 in left eye. Slit lamp biomicroscopy of right eye revealed +1 cells in anterior chamber. Fundoscopic examination of right eye showed hazy media with +2 vitreous cells, fovea showed cyst in the centre, severe vasculitis superiorly with surrounding dot blot hemorrhages and vascular sheathing. Inferiorly showed pigmented scar with small retinal nodules, patches of vasculitis and snowball opacities inferotemporally. Left eye fundus revealed few drusen like deposits in macula. His Mantoux and Toxoplasma IgG test was positive. Serum ACE was > 150, normal HRCT thorax. He was started on sulfamethoxazole and trimethoprim combination along with oral steroids.
