Abstract
Zonulopathy and subluxated lens are commonly encountered in our regular practice. A thorough pre-operative work-up helps to detect the signs of zonular weakness and plan the surgery accordingly. There are certain obvious signs of zonular compromise such as gross lens subluxation, phacodonesis, vitreous prolapse and few others. However, there are certain subtle signs which are not so obvious and often missed, if not vigilant. Here we present, identification of those subtle yet important signs indicative of zonular weakness and subluxation of lens in the early course of surgery to avoid inadvertent complications. Visibility of lens equator after injecting mydriatic agents or tilting the eye, seepage of dye into the posterior chamber, difficulty in initiation of rhexis, anterior capsular dimpling and fluctuation of anterior chamber during phacoemulsification are some of the subtle signs which are highlighted in this video.
