Abstract
The advent of small gauge instrumentation has revolutionized current vitreoretinal surgery.Smaller port size with port optimization and smaller sphere of influence make these cutters more efficient. Multi functionality rendered by smaller more dexterous tips helps reduce need for additional instrumentation. We evaluated various modifications of ILM peeling techniques for macular hole surgery using 27-G MIVS. We found that eyes with macular hole index (MHI) ≥0.5 had better visual acuity than those with MHI<0.5, which is consistent with earlier studies,confirming the clinical use of MHI. 27G cutter was very efficient in all the steps including very accurate PVD induction due to small sphere of influence. Multifunctionality enabled use of 27 G cutter for trimming of inverted flap, apposition of flap onto the hole and air fluid exchange, without need for exchange of instruments. All the modifications of ILM peeling techniques achieved good anatomical closure of macular hole.
