VT0309 – Longstanding Iris foreign body to treat or not to treat!

Abstract
A 61 year female came for routine eye examination, with best corrected visual acuity of 6/6 in both eyes. Past records dated 30 years ago showed suspicious intraocular foreign body and advised for x-ray orbit, patient didn’t follow up as she was symptomless. On examination a nebular corneal opacity was seen, with a pigmented foreign body adherent to iris tissue with degenerative changes over its surface, rest of the iris colour and pattern remaining unchanged. A focal cataract was seen underneath foreign body and fundus examination was normal. X-ray didn’t show evidence of metallic foreign body, while ERG showed diminished amplitude of b wave. Surgery for foreign body removal with cataract was planned; on table it was noted to be a non-magnetic stone foreign body with intact anterior capsule. Decision for removal of such asymptomatic longstanding foreign body without any clinical evidence of siderosis is a subject for discussion.
