Abstract
Familial exudative vitreoretinopathy(FEVR) is a heritable vitreoretinal disorder with incomplete peripheral retinal vascularization. This causes hypoxia promoting abnormal neovascularization, which results in complications like vitreoretinal traction,macular drag and retinal detachment(RD). Challenges arise in FEVR-related RD requiring vitrectomy due to the tenacious nature of the vitreous with abnormal attachments. PVD induction is almost impossible in areas beyond the vascular-avascular junction, which can be very posterior. The remaining peripheral vitreous needs close shaving to prevent iatrogenic breaks. In peripheral tractional RD, traction is relieved to the extent possible and vitreous trimmed on its surface. Peripheral avascular retina is lasered with milder intensity burns to prevent vitreous contraction. In FEVR-related rhegmatogenous RD, vitrectomy with endotamponade is combined with encirclage band which helps to support the vitreous base and peripheral ischemic retina.
