Abstract
Intra-ocular lens (IOL) drop or posterior dislocation of the IOL during cataract surgery is a rare occurrence. It is an undesirable complication for any cataract surgeon. Cases with intra-operative IOL drop are usually referred by the cataract surgeon for further management to a retina surgeon. The cataract surgeon can perform a limited anterior vitrectomy and secure the section to facilitate further management by the retina surgeon. However, attempting the placement of a second IOL in the presence of an already existing IOL drop is not a prudent option. This may lead to further complications. We, hereby, present a video on the rare occurrence of two foldable IOLs in the vitreous cavity and its subsequent management. This was first noted during a pre-operative evaluation. Both the foldable IOLs were explanted one after the other following PPV without any further intra-op complications. A secondary IOL was placed in the sulcus. The patient had a good postoperative recovery of vision.
