Abstract
Foldable IOL dislocation during phacoemulsification often necessitates IOL explantation through an enlarged clear corneal incision. This may lead to hypotony, anterior chamber collapse, corneal endothelial injury, and incision-related astigmatism. Placing a single-piece foldable IOL in the sulcus after retrieving it from the vitreous cavity is very challenging as compared to a 3-piece IOL. Adequate sulcus support is a prerequisite for sulcus placement. Visualization of the sulcus during surgery is often difficult due to intra-op miosis. We describe a novel technique of using iris hooks intra-operatively for visualizing the sulcus while placing a single piece foldable IOL after retrieval from the vitreous cavity. This obviates the need for an IOL explantation. This video demonstrates the successful placement of a foldable single piece posteriorly dislocated IOL in the sulcus using a technique that can be reproduced easily. The patient had good postoperative recovery of vision.
