Cysticercus cellulosea is the causative organism for cysticercosis. It is the larval form of Taenia Solium. Through the hematogenous route it travels to various organs and appears in encysted form in central nervous system, muscles, subcutaneous tissue and rarely eyes. Acquired strabismus, painful proptosis, recurrent episodes of redness are some of clinical presentation. Patient came with the complaint of no improvement of vision post cataract surgery unilaterally after he was operated for mature cataract. On enquiry patient gave the history of recurrent episodes of redness and pain. At presentation his vision was PL with yellow glow of fundus visible. On orbital imaging a well defined cyst with hyperechoic capsule and hypoechoic fluid was seen. On neurological imaging multiple intracranial cysts were found. Patient was started on albendazole with steroid cover. An high index of suspicion is necessary for diagnosis and systemic evaluation is necessary to prevent severe complications.
