Abstract

Superior ophthalmic vein thrombosis (SOVT) is a rare vision and life-threatening complications. It is associated with many underlying etiologies, such as inflammatory orbital disease, trauma, neoplasm, and a hypercoagulable state. Risk factors include at least one of Virchow’s triad of vascular damage, stasis, or hypercoagulability.

A 56-year-old male, presented to the Ophthalmology OPD with chief complaints of swelling in the right eye, pain in the right eye and diplopia since 15 days. VA was 6/6 in both eyes on presentation. He developed diminution of vision in the RE which progressed gradually to CF 3m in 2 months. Anterior segment showed chemosis and congested sentinel vessels on the conjunctiva in the RE. Proptosis evaluation- RE =18mm and LE =10mm. IOP in RE was 25.7mmHg and LE was 18.9 mmHg. MRI and DSA scan showed SOVT with extension into the cavernous sinus and type B carotid-cavernous fistula on the right side. He underwent embolization for the same. Post op vision was 6/12.
