Abstract

A 76-year-old male presented with sudden DOV OS for 1 day (BCVA CFCF). Fundus examination OS showed arteriolar narrowing with boxcarring, diffuse background pallor with small area of normal-appearing retina temporal to optic disc, corresponding to patent cilioretinal arteries. SD OCT showed diffuse inner retinal hyperreflectivity except in the small area corresponding to the patent cilioretinal arteries. OCT angiography OS showed extensive flow void areas at the posterior pole sparing temporal juxta papillary retina corresponding 3 cilioretinal arteries.

At 2 month follow up fundus OS showed disc pallor with normal retinal background. SD OCT showed retinal thinning sparing the area supplied by cilioretinal arteries. OCTA OS showed restoration of flow signals.

Ours is the first case report of CRAO with triple cilioretinal artery sparing. Non invasive imaging using OCTA is critical in establishing early diagnosis and determining prognosis.
