Abstract
A 41 -year-old lady presented with diminution of vision, redness and vision in both eyes of acute onset. Ocular examination revealed bilateral Granulomatous anterior uveitis with optic disc swelling, mild vitritis and multifocal choroiditis. Detailed systemic evaluation showed predominantly normal preliminary evaluation. further testing with rheumatologist guidance showed elevated Serum angiotensin converting enzyme and tuberculin skin test was negative. Computed tomography scan of the chest showed atelectatic bands in apical and posterior segment of right upper lobe and superior and lateral basal segment of right lower lobe; few calcified granulomas in right upper lobe and superior segment of bilateral lower lobes.
Oral steroids resulted in resolution of ocular inflammation and resolution of disc granuloma. Right eye currently showed inflammatory CNVM.
We present this case to highlight the importance of detailed systemic examination in treating cases with multiple confounding factors
