Abstract
Retrospective description of clinical and Multimodal findings of 4monocular patients with atypical SO.
Presenting VA in sympathizing eye ranged from HMCF to finger counting.All eyes had AC reaction and diffuse vitritis.Fundus showed peripapillary fibrosis and peripheral choroiditis.SS OCT showed subretinal uniformly hyperreflective material,outer retinal layer disruption and variable choroidal thickness.FFA showed hyperfluorescent choroiditis lesions and staining of fibrosis and ICGA showed hypo spots. Histopathology of enucleated eyes showed granulomatous inflammation in retina and collagen deposition.Patients were treated with systemic corticosteroids and immunosuppressive therapy/biologicals and remained stable with a final VA ranging from 2/60 to 6/36 at 3m follow up. Diagnostic difficulties,rapid progression,and poor response to steroids/immunosuppressant pose challenge in these atypical cases of SO. Early institution of targeted biologics might prevent these eyes from going blind
