Abstract
introduction:Zonular weakness or dehiscence can cause anterior subluxation of the lens and secondary angle closure glaucoma which can mimic an acute primary angle closure attack.The management of two conditions is different and in secondary angle closure induced by lens subluxation, the treatment is primarily directed towards removal of the crystalline lens. METHODS:5 eyes of 5 patients initially diagnosed as primary angle closure glaucoma were later found to have an occult lens subluxation either before or during cataract surgery. All the patients underwent lens based surgery to remove the cause of angle closure.Biometric features of affected and fellow eyes, postoperative IOP and requirement of glaucoma drugs was noted in all patients. Result: significant difference was found in AC depth of affected eyes and fellow eyes (p=.005).CTR was implanted in 3 patients and SFIOL done in one. there was significant reduction in glaucoma drugs pre and postoperatively (p=.003)
