Abstract
INTRODUCTION-Toxoplasma gondii is the most common cause of infectious retinochoroiditis worldwide.
It typically presents as a chorioretinal scar in inactive phase and a necrotizing chorioretinitis with
overlying vitritis in active phase.Optic nerve involvement is a very rare presentation with absence of classical clinical picture of toxoplasma retinochoroiditis.
We report a case of juxtapapillary retinochoroiditis in a case of u/l optic disc granuloma.Ocular investigations–FFA:late disc leak,OCT:disc hyper reflectivity with surrounding SRF.B scan:homogenous lesion of the disc.Blood investigations:high ACE,16:129 anti-toxoplasma Ig M with -ve Ig G s/o acute infection.Triple therapy and 3 IVI Clindamycin 1 mg and Dexamethasone 100 microgram were given.The lesion started resolving by day 8 with improvement in UCVA from 6/36 to 6/9 by day 16.
CONCLUSION-This disease typically involves the macula but in this case retinochoroidal involvement surrounding ONH can be mistaken for disc edema.
