Abstract
Purpose: To test the efficacy of Y splitting procedure of inferior oblique muscle to mitigate the anti-elevation syndrome.
Methods: A prospective interventional study was conducted to discover the efficacy of inferior oblique muscle Y-splitting procedure in alleviating its anti-elevation effect following its anterior positioning. The reduction in primary gaze hypertropia, excyclotorsion, and anti-elevation were noted at baseline and 6 months.
Results: The average age of 21 subjects was 24 years. The pre-operative hypertropia, excyclotorsion and inferior oblique muscle over-action was reduced significantly in all eyes (p<0.05) at the end of 6 months. Amongst the 21, 16 patients experienced a complete reduction in anti-elevation effect and remaining 5 in a variable amount. Conclusions: In routine inferior oblique muscle surgeries with anterior positioning, it can be split in a Y fashion to achieve all benefits of inferior oblique surgery with mitigated anti-elevation syndrome.
