Abstract
Bilateral Acute Depigmentation of the Iris (BADI) is defined as an acute, symmetric and bilateral depigmentation of the iris stroma without transillumination associated with deposition of pigment in the anterior chamber and in the iridocorneal angle. 30 year old female who had undergone photorefractive keratectomy in both eyes 45 days back and was on tapering dose of combination of moxifloxacin and dexamethasone as postoperative regimen, presented with mild pain, redness since 2days. Her vision was 6/6 blur in both eyes. Examination revealed bilateral pigment dispersion on corneal endothelium (OD>OS)and the anterior chamber with loss of normal iris pattern. Intraocular pressure measured by tonopen was 29mmHg and 25mmHg in OD and OS respectively. Gonioscopy showed open angles with dense pigmentation. Diagnosis of BADI was considered. BADI is an important cause of pigment dispersion and clinicians must be observant for this condition to avoid unnecessary diagnostic tests and treatment.
