Abstract
A 77-year-old one-eyed female presented with progressive DOV OS for 1 month. She gave history of multiple surgeries,including PPV for post-op endophthalmitis following cataract surgery 5 months ago;following which she developed phthisis OD.Anterior segment OS showed granulomatous uveitis & advanced cataract.FFA & ICGA showed early & late hyperfluorescence & prominent disc staining,with corresponding early & late diffuse hypercyanescence.B-Scan USG showed diffuse retinochoroidal thickening with exudative RD.SD-OCT showed obscured choroidal vascular architecture with RPE undulations.A diagnosis of chronic sympathetic ophthalmia (SO) was made.Intensive topical steroid Rx and systemic immunosuppressive Rx were given.3 weeks later,signs of resolution of chronic SO were seen on repeat FFA,ICGA & OCT. SO may present at a chronic stage of disease with varied manifestations. Multimodal imaging comes to the rescue for clinching the diagnosis and timely management of this challenging condition.
